Registering as a patient

If you are new to the area and would like to register at our surgery, you can do this via your preferred method:

• By visiting NHS – Find a GP and completing the online form which will then be sent electronically to the surgery
• Complete our downloadable registration form Spreadsheet version: registration form or our PDF version registration form and once complete send this via email to ssicb-nst.parkmedical@nhs.net
• In person at our main reception desk where we have registration forms available, please speak with our receptionist.
