Sick notes

To request a sick note you can contact us in any of the following ways:

• Online: Use Accurx, available 24hours a day, 7 days a week 

• By phone: Call 01538 399152, 8:00am to 6:30pm

• In person: Speak to our receptionist, 8:00am to 6:30pm

We will ask why you need the sick note, the start and end dates. If you have not been seen before, we may book you an appointment.

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
