Appointments
All requests urgent or routine to see or speak to, a GP or Nurse Practitioner are to be made by completing the appointment form “I want help for a medical issue”, which can be accessed here https://florey.accurx.com/p/M83071. This is link is available during practices opening hours.
Patients will also be able to use the “I have an admin query” to make enquiries about fit notes, order prescriptions, request blood test results, chase referrals and other related issues to save them having to telephone the practice.
Any patient who does not have access to the internet or has difficulty will be able to contact our reception team who will complete the form on the patient’s behalf.
The main advantages of using the appointment form are:
- All urgent problems can be identified and dealt with appropriately with a call or an appointment.
- Simple requests that need telephone advice (or other information to be provided) can be addressed promptly.
- Symptoms that can be assessed and treated in specific clinics (e.g. by our nurse practitioners or clinical pharmacists) can be booked with the right person.
- If the GP/Nurse Practitioner needs to see you in person, they will book an appropriate appointment.
Please ensure you read carefully through the appointment form and follow the advice given. (eg. 999 symptoms)
Those who cannot use online forms because of difficulty with English, with reading or writing, or with using technology (smartphone, tablet or computer) can contact reception for assistance, and will be supported to complete the forms or the receptionist will complete a form on their behalf. Patients completing the appointment form will be able to specify there preferred method of communication, and also best times for the surgery to contact them.
To help ensure the telephone line to the practice is as accessible as possible for the more urgent cases we are asking all patients who have a smartphone, tablet, laptop or PC to use the online appointment or admin forms rather than telephoning and getting the reception team to complete them. All forms are dealt with in the order they come in and getting a receptionist to complete it will not mean it is dealt with any quicker.
With the online form system for either appointments or admin request we will aim to deal with all requests within 2 working days.

Disabled Access
All clinical rooms in our Practice are readily accessible to patients in wheelchairs and the patients’ toilets are designed to accommodate those with disabilities. Please ask for any assistance you may need.

Fit Notes (also known as Sick Notes)
If you are off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification (SC2 form). If you’re off work sick for more than 7 days, your employer will usually ask for a fit note (or Statement of Fitness for Work) from a GP or hospital doctor. Fit notes are sometimes referred to as medical statements or a doctor’s note. These can be requested on the online form under “I have an admin query” Please note if you have not been seen regarding the issue you are currently off with the GP may request an appointment to discuss.
